The State of Black Health Survey is now open for African American, African, and Caribbean residents of Montgomery County through August 28, 2026. The survey, available in English, French, Haitian Creole, Amharic, and Spanish, is intended to inform future health programs and services for Black communities in the county. It takes a few minutes to complete.
Montgomery County invites you to participate in the State of Black Health Survey for Montgomery County. Your voice and experiences are important in helping the MoCo Department of Health and Human Services better understand the health needs, concerns, and priorities of Black and African American communities.
The information gathered through this survey will help inform the planning and delivery of services in the years ahead. The more community members who participate, the better we understand the issues that matter most and advocate for meaningful improvements.
